Hiring Paperwork Instructions—Working in Canada

THESE INSTRUCTIONS ARE FOR CANADIANS WORKING IN CANADA.


Once you have received an e-mail offering you a position, you must complete the required organizational and federal hiring paperwork. Please complete all forms necessary and submit them to our office within two weeks of receiving a job offer. The e-mail you received should have told you what forms to turn in. Please call (801) 422-3817 x 0 or e-mail Leanne, leanne.olson@byu.edu, with any questions.


IMPORTANT NOTE: If paperwork is received incomplete or incorrect, you will be required to fill it out again and re-submit it to our office. Read the paperwork instructions carefully.


Documents Especially for Youth Programs needs from you:

(Please click on each document for instructions on how to fill them out)

  1. TD1 Form (send original please)
  2. TD Form for our province (send original please)
  3. Direct deposit (to a Canadian bank)
  4. Social Insurance Card, Copy of (signed)

Helpful Hints

Here are some Do´s and Don´ts regarding your paperwork. Theses are very important and if not followed will result in needing to re-submit your paperwork corrected.


Do´s Don´ts
Use Blue or Black Ink Do not use pencil or other colored ink
Print Clearly Do not write in cursive (excluding signatures)
Put a simple line through errors (mistake) Do not scribble or black out errors / use white out
Spell out complete words Do not use abbreviations

You have 2 ways to turn in your paperwork: Mail or In Person

  1. Mail: Since you are sending confidential paperwork with your personal information, we recommend you send it with some type of tracking method.

  2. In Person: If you are going to be in the Provo area, you may leave your paperwork with the receptionist at the front desk.
