I-9 Form Instructions

This is a required US Government document. It is your responsibility to fill out only Section 1 completely, including signing, dating and marking the appropriate boxes. If you neglect signing and dating this form, it will be invalidated, so please do not miss this very important detail. Section 2 can only be filled out by a full-time CES employee that is seeing your original documentation. If you are in the Provo area, you may bring it into the EFY office and any full time staff is able to fill is out for you. Again, they will need to see your original documentation. We have information written below for CES employee’s to aide in filling out this form.

DO NOT FILL-IN SECTION 2 INFORMATION FOR THE CES EMPLOYEE. ALL OF SECTION 2 MUST BE COMPLETED BY A CES EMPLOYEE ONLY, AND NO-ONE ELSE.

CES EMPLOYEE Guide for Section 2
Please be sure to fill out list A if the applicant brings in their passport. Otherwise leave A blank and fill out both B and C. DO NOT fill out all three, that will invalidate the form.

Please take the I-9 and your original documents (e.g. passport, SS card, Drivers License, etc.) to your nearest CES Full-time employee (institute director, instructor, secretary/staff or a seminary teacher). Documentation filled out by volunteers cannot not be accepted. Please refer to the sample I-9 for guidance on how to fill this form out. Section 3 please leave blank.

You will see below this paragraph the link to print out the I-9 form. There is a second link which shows three examples of our most commonly used documentation in the “Sample I-9 Form” section. Please look here to ensure that it is correctly filled out the first time. Counselors, by doing this correctly the first time, you will not have to spend as much time waiting to hear what sessions you have been hired for. International Students/Instructors may not need to fill out an I-9 if they are not working at a session in the United States. Before filling out this document, please contact (801) 422-5023 to see if this is a necessary document.
