W-4 Form Instructions

This is an IRS employment form required for tax purposes.

Please note: BYU student employees are required to submit this information as you are paid for employment at EFY by the Church, not Brigham Young University. When completing this form, please use the address where your W-2 Form should be mailed to you next January.

When you get to line 5, please populate line 5 and 6, OR 7, but not both. Lines 5 and 6 represent one type of tax liability, and line 7 specifies a different amount of tax liability. If you would like further assistance, please contact your financial consultant.

The directions on the W-4 should guide you correctly based on your personal information.

Note: Make sure you complete a current W-4 Form. When you sign and date the form, the signed date must match the year at the top of the W-4 form (i.e., don't sign and date a 2010 [or earlier] W-4 Form). It must be titled W-4 Form (2011).
