Before continuing in the hiring process, you must have been contacted by our department. For any questions, please call (801) 422-3817.

Once you receive an e–mail inviting you to turn in hiring documents, please complete all forms necessary and submit them to our office within two weeks of receiving the hiring documents email. Remember, you are not officially hired until your required hiring documents are correctly and completely filled out and received by the HR Office at BYU. The e-mail you received should have told you what forms to turn in. Please call (801) 422-3817 or e-mail efyhiring@byu.edu with any questions.

IMPORTANT NOTE: If paperwork is received incomplete or incorrect, you will be required to fill it out again and re-submit it to our office. Read the paperwork instructions carefully.

Attention BYU Students: If you have been hired and asked to submit paperwork for EFY and have a job on campus, you will still need to submit an online W4 and direct deposit through the applicant home page for EFY. Payroll for EFY is completed through the Church Office Building and information from BYU can be supplied only with your completion of the following tasks.

  • Direct deposit – Go to your applicant home page, click on the direct deposit link. If you have an active BYU student job, you will see your account information there. Please be sure to click “change” and then save to create a batch to be sent to the Church Office Building. Without clicking “save”, your payroll will be delayed.
  • W4 – Go to your applicant home page, click on the W4 link. You will see the information already present in the BYU system. By clicking “Submit”, you will be able to create a batch for the Church Office Building. Without clicking “submit”, your payroll and W2 at the end of the year will be delayed.
  • If you are a BYU student, haven’t worked on campus before and haven’t worked EFY before, you may need to complete all paperwork. Please contact 801–422–3817 to verify what is needed.

    W–4 Form

    The W–4 is a required IRS form that all places of employment must have filled out. The W–4 Form is now submitted online. To fill out your W–4, please go to the HRMS page and log in with your Route Y credentials, then click on the W–4 link. Even if your information is already there, you MUST click "submit". Otherwise, your information will not be received. If you have any questions or are not able to complete and submit the form, please call (801) 422-3817.

    Direct Deposit

    Direct deposit is mandatory and is also done online. To submit your Direct Deposit information online, please go to the HRMS page and log in with your Route Y credentials, then click on the Direct Deposit link. Even if your information is already there, you MUST click "change" then "save". Otherwise, your information will not be received. If you have any questions or are not able to complete and submit your direct deposit, please call (801) 422-3817.

    Social Security Card

    Please supply a copy of your SIGNED social security card with your hiring documentation. Without this, your file cannot be complete as this is mandatory BYU policy to keep on file.

    If you do not have your social security card, you can log on to the internet to the “Social Security Administration” to find the nearest office to you. When you go in to apply for a new card, a receipt will be given to you. Save the receipt they give you and use this for a temporary hire. Once the new card comes in the mail, sign it and send a copy of it to us right away.

    PLEASE BE SURE TO SIGN YOUR CARD BEFORE MAKING THE COPY TO SEND TO US.

    I-9 Form Instructions

    The I-9 is a mandatory Department of Homeland Security form that MUST be filled out prior to working for EFY, which verifies your eligibility to work in the United States. If asked to fill out this form, please do the following:

    1. Print the blank I-9 form using this link. Review this form as there are 3 sections to it.
    2. As the employee, fill in section 1 ONLY.
    3. Take the form with the appropriate ORIGINAL ID’s to have the employer fill in section 2.
      1. If you are able to come into the EFY office on BYU campus, please contact our office at 801-422-3817 to set up an appointment.
      2. If you are not able to come to the EFY office, you will need to locate a full-time paid CES employee more close to you to have section 2 completed.
    4. Please leave section 3 blank.
    5. Mail (UPS and FedEx are recommended) the completed form into the EFY office ASAP with tracking using the following address.

    6. Once this information has been successfully entered into the computer system, you will then be invited to accept the offered sessions. If you do not get an email within 14 days of mailing, please contact our office at 801-422-3817 to inquire the status as mail can sometimes be unpredictable.

    If you live in the Provo area and would like a member of our office to assist you with filling out section 2 of the I-9, we would be happy with doing so. This option is done by appointment only by calling (801) 422-3817.

    International Applicants may not need to fill out an I-9 if they are not working at a session in the United States. Before filling out this document, please contact (801) 422-3817 to see if this is a necessary document.

    Direct Deposit Instructions

    New direct deposit information is required each year. When filling out this form, you will notice the verbage on this form is for American banks that use a 9 digit routing number. Canadian banks use institution numbers which are 3 digits long and transit numbers which is 5 digits long. Here’s an example PDF file.

    How to fill out the form for the current calendar year:
    1. Print the direct deposit form.
    2. 2.) Fill in the contact information, account number, institution and transit numbers, sign and date. Please note: it is possible to have direct deposits go to more than one account.
    3. Attach a voided check to a Canadian bank or bank verification.
    4. The preferred submittal method is to scan and email to efyhiring@byu.edu or fax to 801-422-0731. You could mail your documents to our office but please be advised may take up to 2 weeks to be received and using a method with tracking is advisable. Our mailing address is as follows:

    One note of caution � please be sure that everything written is legible and if you have any changes in your bank accounts, a new form will need to be completed ASAP to prevent any payroll delays.

    2. Fill in the contact information, account number, institution and transit numbers, sign and date. Please note: you must END the old account information and BEGIN the new, so both bank accounts will be noted on this form.

Helpful Hints

Here are some Do’s and Don’ts regarding your paperwork. Theses are very important and if not followed will result in needing to re-submit your paperwork corrected.


Do’s Don’ts
Use Blue or Black Ink Do not use pencil or other colored ink
Print Clearly Do not write in cursive (excluding signatures)
Put a simple line through errors (mistake) Do not scribble or black out errors / use white out
Spell out complete words Do not use abbreviations

You have 2 ways to turn in your paperwork: In Person or Mail

  1. In Person: You are welcome Monday–Friday in between the hours of 8:00 a.m.–12:00 p.m. MST or 1:00 p.m.-4:45 p.m. MST.
    1. If your I–9 is already complete, you may leave it with the receptionist at the front desk
    2. If your I–9 is NOT complete, be prepared and bring the original documents you want to use to have section 2 of the I-9 form completed. Only a full time Especially for Youth Programs employee can fill out your I–9.
  2. Mail: Since you are sending confidential paperwork with your personal information, we recommend you send it with some type of tracking method. IF MAILING WITH SPECIAL TRACKING TO EFY, PLEASE USE UPS OR FED EX.

  1. E–mail: Scan your paperwork and e–mail it to efyhiring@byu.edu.
  2. Fax: Fax your paperwork to 801–422–0731.
  3. Mail: Since you are sending confidential paperwork with your personal information, we recommend you send it with some type of tracking method. WHEN MAILING WITH TRACKING INTERNATIONALLY, PLEASE BE SURE TO MAIL VIA DSL.
