Accommodation Request Form

High school students with special learning needs should work with their school counseling office to ensure appropriate accommodations are made. If counselors need specific assistance from Independent Study in providing appropriate accommodations, counselors may contact our office at 1-800-914-8931. However, accommodations which compromise the essential functions of a course will not be granted. In addition, BYU Independent Study does not authorize the following without petitioning:

  • multiple attempts on an exam beyond the normal retake policy
  • open book/notes tests (unless specified open book by the instructor for all students)
  • altered test forms

To petition any of the above restrictions, or request specific assistance, counselors should fill out our Counselor Accommodation Request Form. Documentation of disability may be required for some requests.

The following accommodations are not authorized for any reason:

  • use of proctors not certified by our offices
  • retroactive accommodations
  • grades based on effort

Home-schooled students and university students may request special accommodations by submitting a completed request for accommodations form with a valid documentation of disability. An Individual Education Plan (IEP) submitted for consideration must be less than three years old, and processing time must be allowed to provide for special requests and accommodations.
