Application Process:

Fill out the application form (APPLICATION link) and the ecclesiastical endorsement form (ENDORSEMENT link).
The ecclesiastical endorsement you will need to work as a youth mentor is in addition to all other ecclesiastical endorsements you may currently have (temple recommend, BYU student).

Do not take the ecclesiastical form to your bishop; simply submit it with your application.

In most cases you will not need to meet with your bishop or stake president in order for them to endorse you for this position. (If you are currently working with your bishop on an issue, you will not be able to receive the endorsement necessary to work at this time.)
