
Cost
The $8,000 tuition for BYU's Six Sigma Certificate Programs (Black Belt, Master Lean) includes all texts and materials, Web site support, parking, certificate upon completion, and documented CEUs from Pacific Rim Business Review, Inc.
Location
All classes will be held at the Brigham Young University Conference Center in Provo, Utah. Additional information including a map will be provided upon registration with your confirmation packet. Admission and Standards
Participants of any race, color, creed, and ethnic or national origin are accepted for admission to this program provided they maintain the university standards while on campus. These include modesty in dress and conduct and abstinence from alcohol, tobacco, coffee, tea, and harmful drugs.
We are committed to providing a learning atmosphere that reasonably accommodates persons with disabilities who are otherwise qualified. Prior to registration, applicants with disabilities are invited to discuss the program content and available facilities with the program director.
Payment Options
Participants may pay the registration fee in full at the time of registration, or they may choose to participate in a multipayment plan. Individuals using the multipayment plan must provide a minimum payment of half of the tuition. They must also sign a multipayment plan agreement. Participants using the multipayment plan will be mailed the payment agreement once the initial payment has been received and initial registration has been processed. The final balance is due at the start of the program. Questions concerning the multipayment plan can be answered by calling (801) 422-6760.
Refunds, Substitutions, and Cancellations
A $100 fee will be assessed for each refund requested. All refunds must be requested in writing and received in our office no later than (July 6, 2007) two weeks before the program begins (by 5:00 p.m.). Please submit any cancellation requests by e-mail at cw136@byu.edu or mail them to BYU Conferences and Workshops, 136 Harman Continuing Education Building, Provo, UT 84602. No refunds can be given beyond the two-week deadline.
Participants using the multipayment plan will be billed for and are liable for the entire registration fee. You may at any time arrange for a substitute to come in your place without penalty. However, this substitute must be identified to BYU before the program begins. Participants will receive a full registration refund if the program is canceled for any reason. However, the university cannot be responsible for any cancellation or change charges assessed by airlines, travel agencies, or other institutions.
