The Utah County Hospice Coalition has been formed by a committed alliance of local independent hospice agencies that are united in the quest to provide professional, quality care to every deserving patient who qualifies for hospice and desires to have the care and support offered by this powerful program of services. We believe that every individual, and their family, dealing with an incurable illness deserves the opportunity to make informed decisions.

Our goal in uniting together is to educate, inform and motivate physicians, clinicians, patients and families to understand, promote and utilize the invaluable care and resources offered by hospice.

Note: Registration is limited. We encourage you to register early.

  3. Mail the completed registration form and payment to BYU Conferences and Workshops, Utah County Hospice Coalition, 120 Harman Continuing Education Building, PO Box 21514, Brigham Young University, Provo, UT 84602.

Cancellations and Refunds
A handling fee of $20 will be assessed on all refund requests received by 5:00 p.m. Friday, September 7, 2007. No refunds will be available after this date. Mail requests to Utah County Hospice Coalition Workshop Refunds, 147 Harman Continuing Education Building, Provo, UT 84602.

Admission and Standards
Participants of any race, color, creed, and ethnic or national origin are accepted for admission to this program provided they maintain the university standards while on campus. These include modesty in dress and conduct and abstinence from alcohol, tobacco, coffee, tea, and harmful drugs.

We are committed to providing a learning atmosphere which reasonably accommodates persons with disabilities who are otherwise qualified. Prior to registration, applicants with disabilities are invited to discuss both the program content and available facilities with the program director.

* Timpanogos Regional Hospital is accredited by the Utah Medical Association to sponsor continuing Medical Education for Physicians. Timpanogos Regional Hospital designates this educational activity for a maximum of 6 hours in category 1 credit towards the AMA Physician's Recognition Award. Each physician should claim only those hours of credit that he/she actually spent in the education activity. CEU's and CME's have been approved for 6.5 hours for Nurse Social Workers. NASW has approved 6.5 hours of Continuing Education. Utah Nurses Association has approved 6.5 continuing medical hours for nurses.
