Description:                              Sometimes elderly family members need help organizing their

important papers so they are readily accessible when needed.  Tips on giving assistance include the following:

!                One-page medical reference sheet containing all medications, allergic reactions, a brief summary of medical history with dates, primary doctors, other contracts, and insurance information.  This information, along with a copy of a living will and medical treatment plan are kept together in case of a medical emergency.

!                Blood pressure sheet for use when health problems are constant to give doctors an overview of what may be occurring with the heart.

!                A record of appointments can be kept on a calendar.

!                Family members names, birthdays, addresses, and phone numbers are kept together to assist with making contacts, sending birthday cards, and for family activities.

!                Family members rotate weeks for having activities with the elderly family member to lift their spirits.

This chart is to help in organizing Family Aid for an elderly family member.  If the elderly family member lives at home, then family members can come in and assist during their week and check in regularly to make sure things are going well.  If the elderly person lives with a family member, it is a chart that can give the elderly member time with other family members and give the caretaker time off for activities needed.
